I knowingly and willingly consent to participate in Pleasurebent Tours tours, activities, and events (“Tour Activities”) during the COVID-19 pandemic. *

I understand the COVID-19 virus has a long incubation period during which carriers of the virus may not show symptoms and still be highly contagious. It is impossible to determine who has COVID-19 and who does not have it given the current limits in virus testing. *

I understand that due to the proximity of tour participants, staff, and the public with whom I may encounter during Tour Activities and the characteristics of the virus, that I may have an elevated risk of contracting the virus simply by attending and participating in Tour Activities with Pleasurebent Tours. *

I confirm that I will not attend any Tour Activities if I, or any member of my household, am/is exhibiting any of the following symptoms of COVID-19 listed below, and in the event I develop any of the following symptoms of COVID-19 listed below after attending a Tour Activity, agree to immediately notify a representative of Pleasurebent Tours:  Fever – Temperature: 100.4 degrees  Shortness of Breath  Loss of Sense of Taste or Smell  Dry Cough  Runny Nose  Sore Throat *

I understand that Pleasurebent Tour’s safety guidelines are based upon guidelines and information provided by the Center for Disease Control, and any governmental entity or agency in whose jurisdiction Tour Activities are being conducted, and are subject to revision based upon new information and/or government requirements. *

I knowingly and freely assume all risks, both known and unknown, related to possible exposure to and illness from COVID-19 arising out of or related to my attendance of any Tour Activities hosted by Pleasurebent Tours. I, for myself and on behalf of my heirs, assigns, and personal representatives hereby release and hold harmless Pleasurebent Tours, its officers, directors, agents, employees, and other participants of Tour Activities (“Released Parties”), with respect to any and all illness, disability, death, loss or damage to person or property, whether arising from the negligence of the Released Parties or otherwise, to the fullest extent permitted by law. *
