U.S. Adult CF Association Lauren Melissa Kelly Scholarship Application for Fall 2019 Semester
CF Roundtable offers the Lauren Melissa Kelly (LMK) Scholarship award each semester, in honor of Lauren Melissa Kelly. The academic scholarships of up to $2,500 are awarded to adults with cystic fibrosis who are pursuing career certifications, associates, and bachelor and graduate degrees. Please see below for the LMK Scholarship Criteria.

CF Roundtable is completely independent of the Cystic Fibrosis Foundation. We are uniquely geared toward adults 18 years old and over with cystic fibrosis via CF Roundtable programs. CF Roundtable is a non-profit that relies on grants and community donors to provide various services to the adult cystic fibrosis community.

The scholarship application below will take approximately an hour to complete and requires proof of documentation for certain sections (transcripts, etc.). Please fill out the form yourself. The bulk of the application comprises three essay questions. Below you will find the essay questions and list of required documentation so you can plan accordingly before starting the application form. It's highly recommended you have these ready to submit before starting the application process as you can't save your work and return.

Essay questions are as follows:

Essay I - Personal Achievements and Leadership: Describe how you have demonstrated leadership ability, both within the CF community and elsewhere. How does CF impact your various personal achievements?

Essay II - Financial Need: From a financial standpoint, what impact would this scholarship have on your education? Discuss any special personal or family circumstances affecting your need for financial assistance.

Essay III - Background/Influence: Who in your life has been your biggest influence and why? How has CF contributed to who you are today?

Essay IV - Future Plans and Goals: Briefly describe your short and long-term goals. How will receiving this scholarship aid you in your academic and career journey? How does CF factor into these goals?

In addition to the essay questions, you'll need to submit the following documents to scholarships@cfroundtable.com and upload to the form to complete your application:

Please title each document with your name followed by document type (e.g. Jane Doe, Bio or Jane Doe, Essays).

1. Proof of Enrollment (PDF preferred)
2. Relevant school transcripts (PDF preferred)
3. Resume (PDF preferred)
4. Headshot (JPEG, high resolution for use in our newsletter and various social media outlets)
5. Proof of CF Diagnosis
6. Word document with all three essay questions

Winners will be notified via email within 30 days of the close of every semester application period. Winners will also be tagged/notified in our various social media outlets.
