This form should only be used by residents of 𝗦𝗰𝗵𝘄𝗮𝗯 and 𝗝𝗮𝗰𝗸 𝗠𝗰𝗗𝗼𝗻𝗮𝗹𝗱 𝗛𝗮𝗹𝗹 for network registration requests for iOT devices only (Amazon Echo, Apple TV, Playstation, Xbox, etc.).
One form submission is required for each device registration.

Computers and mobile devices should be added through the self registration process when you are on campus by connecting to the "Stanford" WiFi which will open your default browser and takes you to the self-registration page.
