This form is to gain information about persons who would like to perform. You will be contacted by our Performance coordinator to learn more about our event and the terms and conditions. Professional performers will be expected to perform several times over a 2-3 hours period  between 1 to 830 pm on November 24th.  Our event needs will control the timing and placement. Performers will be expected to attend one meeting prior to the event that can be either in person or by zoom.

FOR NONPROFIT organization performers, please name the organization.
FOR NONPROFIT organization performers - you may have a tent and table in the footprint. The tent has to be supervised by adults if minors participate or are members of the performance group.  Are you requesting a space for a tent? 
A certificate of insurance is required for professional performers and nonprofit organization performers showing the Downtown  Alliance and the City of Fayetteville as additional insureds. Can you have your certificate ready by November 11?  *
The manager of the group must participate in at least one meeting in November with the Dickens committee. This meeting will be either in person or by zoom. Do you understand and agree to meet.
