2021 Academy of LDS Dentists Scholarship Application Form - Recent Graduates
ANNUAL CONFERENCE SCHOLARSHIP: This is a unique scholarship that provides an exceptional opportunity for a recent graduate dentist to receive complimentary conference registration to attend the Academy of LDS Dentists 44th Annual Conference on August 13-14, 2021 in Provo, Utah.

As this is a conference scholarship, please check your schedule and confirm that the dates of August 13-14, 2021 are open on your calendar to attend the 2-day conference. Visit academyofldsdentists.com to learn more about the Academy and the conference.

APPLICATION REQUIREMENTS. TO BE ELIGIBLE FOR THE SCHOLARSHIP, CANDIDATES NEED TO:

1. Be a current recent graduate, dentist or hygienist member of the Academy of LDS Dentists. Recent graduate membership is free through the end of the 2nd year after graduation, but it is required to be eligible for the scholarship. If you haven't already done so, visit academyofldsdentists.com/join and follow the prompts to register as a member of the Academy by May15, 2021;

2. Have graduated from dental school or dental hygiene school in 2019, 2020, or 2021;
