**Families will now receive 2 small cans of fomula (or 1 large can). Families can still sign up every other week.**

Please complete the following form, providing all required information. Requests must be received by 5pm Monday for pick up on Thursday. Requests received after that time will be available for pick up the following week.

You will be contacted by a staff person with a time to pick up items between 3 and 5pm on Thursday afternoons at our office located at 609 West Clinton Street, Ithaca.

Supplies are limited to 24 diapers or 12 pull ups per child and up to 2 small cans of formula per child every 2 weeks.

Looking for ongoing support? Participants in our Family Services program receive individualized parenting support, child development information, and connection to community resources. Baby supplies, transportation support, and so many other services are available. We also have expertise in working with teen parents! Free lactation counseling, postpartum support, and fatherhood programs are available to all participants.

Our programs are open to ANY family in Tompkins or Cortland counties who are pregnant or have at least one child under age 5.

POSTPARTUM SUPPORT:
Postpartum Support is available on the 2nd and 4th Tuesdays of each month from 12-1 via Zoom. Please see our facebook page or website for login information.

LACTATION CONSULTATIONS:
Phone and virtual lactation consultations are available free of charge. Please call (607) 273-0259 and a staff member will return your call.

We just want you to be safe while you're at it. You may choose TWO options from the list below. If you choose just one option, you will receive two packages of that option. Supplies may be requested twice a month.
