2015 Champs Pre-Scouting Form

Due to sabotage of the thread, we've decided to create a form that teams can fill out, and we'll check through the submissions before adding them to the spreadsheet. It will take a while longer since we have to look though them, but hopefully we won't lose data or get as many trolls on it.
