Parent(s)/Legal Guardian(s):  Please complete the "Report Your Child's Absence Form" in its entirety.   If you reported your child's absence via phone or email, you do not need to complete this form.   All doctor notes or court- appointed notifications should be turned into the main office upon your child's return within 48 hours.

This form must be completed each day that your student will be absent.

Students are allowed 10 sick absences per school year, with a parent phone call and/or note. After 10 sick absences, future absences related to illness will be unexcused unless a professional medical note is provided.
