For those who were not able to participate in focus groups with the researchers, we are providing this open-ended questionnaire with most of the questions from the focus group (minus one ice-breaker question).

We appreciate very much you taking a few minutes to give your perspective on the state of adult tennis and specifically on efforts being made by the USTA. We want to encourage you to be as candid as possible here. One, your confidentiality will be protected in the reporting of the data, with no statements being directly attributed to any one individual, and two, representatives of USTA NorCal sincerely want to address the declines in adult tennis participation and can only do so if they fully understand the issue/s at hand.

INFORMED CONSENT TO PARTICIPATE IN A RESEARCH PROJECT: Adult Participation in Tennis

INTRODUCTION
This form asks for your agreement to participate in a research project on adult participation in tennis. Your participation involves engagement with the following online questionnaire. It is expected that your participation will take approximately 10-15 minutes. There are no risks anticipated with your participation. You and others may personally benefit from this study. If you are interested in participating, please review the following information.

PURPOSE OF THE STUDY AND PROPOSED BENEFITS
The purpose of the study is to better understand adult motivation to participate in tennis.

Potential benefits associated with the study include reinvigorating adult tennis players to participate in tennis.

YOUR PARTICIPATION
If you agree to participate, please complete the online questionnaire, at a time convenient to you.

As mentioned, your participation will take approximately 10-15 minutes.

PROTECTIONS AND POTENTIAL RISKS
Please be aware that you are not required to participate in this research, refusal to participate will not involve any penalty or loss of benefits to which you are otherwise entitled, and you may discontinue your participation at any time. If you decide to withdraw your participation, you must let the researchers know. You may omit responses to any questions you choose not to answer.

There are no risks anticipated with your participation in this study.

Your confidentiality will be protected as only the researchers will know the names of those who respond for focus groups, and even the researchers will not know the identity of those completing the following online questionnaire.
