What services will be provided at the booth?
Please detail what types of information, resources, or other services will be available at your booth as well as any giveaways or handouts. Also include any printing or other materials your organization would need SHOW to provide.

Who will be staffing your booth?
Please include the names of those planning to volunteer at your booth on the day of the event. Also include any need for additional event volunteers provided by SHOW. If you are unsure of who will be available to staff your booth, please indicate so below and we will follow up closer to the event date.

What type of equipment does your booth require?
Please details number of tables, chairs, and/or tents your booth requires. A standard booth includes 1 10x10 tent, 1 table and tablecloth, and 2 chairs. Also include any need or preference for electricity, wifi, private or quiet space, etc.

Can your organization provide a certificate of liability insurance?
All organizations will need to send a certificate of liability insurance to healthfair@showaz.org. Additional documentation proving provider licensure may be needed depending on the services provided by the booth.
Will your organization be contributing a gift (~$25 value) for the raffle for event attendees?
Suggested donations include bus tickets, backpacks, new socks, and basic toiletries.
