Use this form below for any Spa Party groups of 3 or more guests interested in coming to Utopia Salon and Day Spa for 2 or more services.
Please list the guests first and last name and services they are interested in receiving. It is strongly recommended to review our pricing on our full service menu on
utopiahawleypa.com.
Once submitted, please allow 24- 48 hours for review. The host will be contacted to discuss availability. Once contract is agreed upon, a 50% deposit will be taken no later than 48 hours after and Service Breakdown sent by email.
Final payment is due in ONE payment 2 days prior to Spa Party.
Food & Beverages
Parties that will be at Utopia for 4 hours or more are welcome to bring a small spread of food and beverages. Must be 21 years of age with valid ID. Glasses provided.
Decorations
-Decoration are not able to be brought into our salon or spa are unless Host chooses full exclusivity for the FULL amount of time that your party will be receiving services. You will be allotted 15 minutes before and after to decorate.
Gratuity
20% gratuity is charged for all party services; additional gratuities are at the discretion of each client.
Cancellation Policy
14 days is required for 100% refund. 7 days is required for 50% refund. Any guests that do not attend their appointments will forfeit their deposit and final payment. Another guest from the party can take the appointment if the schedule allows.
Deposit Requirement
We will contact you 24 -48 hours of sending in this request with a breakdown to your email as well as a phone call to take your deposit. This deposit will be applied towards your final bill on the day of your services.
Deposit can be made in person with cash or check, a credit/debit card on file with a 2% process fee or Venmo with a 3% charge. The deposit will need to be taken within 2 business days of the breakdown being sent.
Additional Services
Services added after the contract is complete will be added as a separate charge to the credit card on file. And will be charged day of the event.
Final Payment Policy
Utopia Salon and Day Spa enters the Spa Party Contract with the Host (not the guests) and therefore the deposit, payments, and fees or discrepancies are ultimately the sole responsibility the Host. These changes and additions must be submitted via email. American Express is NOT accepted.
Final payment will be due 2 days prior to your Spa Party event.
Late Arrivals
Please plan for all members to arrive at Utopia 20 minutes early. If any guests are more than 15 minutes late, our staff may not be able to accommodate them. If your party is more than 30 minutes late without any contact, Utopia Salon and Day Spa will consider your party a “no-show” resulting in 100% forfeiture of all services.
Important Information:
-For any children below 18, there is a release form.
-Relaxation area is a communal space, please understand this space will be used by all guests of the spa.
-Final appointment times will be based off of requested services, staff and service availability and first come, first serve.
