We, the parents of the below-named player, hereby grant permission for them to participate in the Everman Baseball/Softball Camp and acknowledge the fact that they are physically able to participate in camp activities.  We understand that the Everman Baseball/Softball Camp does not provide medical insurance covering injuries of any nature incurred during the 2021 Everman Baseball/Softball Camp.  The undersigned hereby releases the Everman Baseball/Softball Camp, Everman ISD, and Camp Staff from any and all claims, demands, and causes of action whatsoever in any way growing out of or resulting from the participation in the Everman Baseball/Softball Camp.

We will adhere to all Covid guidelines.  Campers must wear a mask when not participating, bring their own water & socially distance when possible.  If parents choose to stay and watch, they will be required to wear a mask.  Each camper, staff, and parent will have their temperature taken when entering the facilities.
