USQ National Qualifier Registration
FILL OUT THIS FORM TO EXPRESS INTEREST IN A NATIONAL QUALIFIER. THE BELOW FORM IS FOR FINAL REGISTRATION.

All teams are required to register for qualifiers to collect information and confirm attendance at the event. This registration is due by January 14. If you plan to attend multiple events, you will need to submit this form for each one by the same deadline. If you must drop from an event at any time (especially after filling out this form), please email events@usquadball.org and cc gameplay@usquadball.org.

On this form, you'll be asked to provide information on
  1. Planned Travel and Housing
  2. Officials, Equipment, and Gameplay
  3. Any additional feedback
