Model Release Form

This form is for the purpose of getting player permission to use their images for Garden of Destinies LARP promotional or advertising materials. It functions as a type of Model Release Form. We intend to use photos in such spaces as: our website, Facebook groups, and other various websites that allow us to upload photographs to demonstrate our LARP.

We are not intending to sell any of your images, and if we wish to do something of that nature (for example, create an official rules book) we will re-contact any persons whose images we wish to use.
