Please describe your organizational structure for the college and career work in your district. Who leads the work, who supports the work, and how are other areas of the district engaged? Please identify who currently works on postsecondary advising in the district. Please send a copy of your organizational chart to alisonr@npsi.org if you have one available.

What does your district hope to get out of participation in the NPSI National Network? What changes would you like to happen in your district?
Other than money, what are your biggest challenges for implementing your college and career framework and strategies?
Identify your team members who will comprise the district leadership team for the Network, including each team member’s title and role (i.e., leadership, data, counseling). Please note who will be the contact person for the team for all communication going forward. 

Teams should consist of a minimum of  three administrators. Team members typically include:

o   District leadership representatives (e.g., Assistant Superintendent, Chief Academic Officer, College and Career Director, Director of High School Programs, etc.)

o   District counseling representative (e.g., Director of School Counseling, Director of Student Supports or Director of Student Engagement)

o   District data representative (e.g., Director of Research and Evaluation, Accountability Director, Data Strategist, etc.)

o   Other senior leadership representatives (e.g., Teaching and Learning, Social Emotional Learning, Family and Community Engagement, Diverse Learners/Special Education, Multilingual Learners, etc.)
