Funding Request Form
Before Creative Media can accept your project request, your budget must be approved by the Office Manager (and the controller when applicable). Please submit this form with as much detail as possible. This information will be be reviewed and you will be notified if your funding request is approved. 

Important Notes:
  • Funding decisions will be made based on the language of the expense line in your budget. 
  • All uniform purchase requests must be made on or before February 2nd, 2026
  • Uniform requests with quantities that exceed the total number of staff/volunteers may be rejected.
  • The last date to submit a Funding Request Form for the 25-26 school year is May 22, 2026
