In order for Adventures in the US to create the perfect itinerary for your get-away, we need to know some information about you and your interests.

The more information you give us, the more personalized your itinerary will be, so please plan to spend about 10-15 minutes completing this questionnaire. If we have any questions about your answers, we will contact you using your preferred contact method indicated in the form below.

We will create a preliminary itinerary once we receive this completed form from you. After that, we will send you the completed itinerary, and if you love it, great, nothing more is needed. But, if you'd like some adjustments, we will schedule a short Zoom call with you to go over the itinerary and learn what areas of the itinerary you would like to see adjustments made, and then we will update it and send you a revised itinerary.
