It is required by Rocky's Skate & Sales LLC, DBA Black Mamba Skate Park / Releasee that this Hold Harmless Release be completed by each participant over 18 or participant’s parent or legal guardian. The mandatory understanding concerning participation is listed as follows:

PART ONE:

1. If participant is under 18 you give your child, identified on the top of this form, permission to participate at Rocky's Skate & Sales LLC, DBA Black Mamba Skate Park and to enter for any purpose any restricted area wherein admittance to the general pubic is prohibited.

2. I/WE HEARBY RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE Rocky's Skate & Sales LLC, DBA Black Mamba Skate Park, participants, officials, skate park employees, sponsors, advertisers, lessees of the premises used to conduct the event and such them their officers, agents and employees, all for the purposes herein referred to as “Releasee,” from all liability to the undersigned, my/our personal representatives, assigns, executors, heirs, including but not limited to death of the participant or damage to property, caused or alleged to be caused in whole or in part by the negligence of the “Releasee” or otherwise.

3. You understand that Rocky's Skate & Sales LLC, DBA Black Mamba Skate Park does not provide any Accident or Medical Insurance and that you are required to provide this for your child. You agree that you are financially responsible for all medical expenses whatsoever.

4. You understand that the terms of this agreement are legally binding and certify that you are signing this agreement of your own free will after carefully reading it.

PART TWO: Medical Authorization

In the event of serious illness or injury, I authorize Rocky's Skate & Sales LLC, DBA Black Mamba Skate Park to call an ambulance to transport myself or my child to a hospital emergency facility for treatment. All reasonable attempts to contact a parent or guardian will be made. I accept responsibility for all costs involved in the transport and treatment.

PART THREE: Facility Rules and Regulations

1. Helmets must be worn at all times by all participants in the ramp area. Use of other protective gear is recommended. (helmet/wrist/elbow/knee pads) Parent(s) and or legal guardian(s) will instruct the minor participant that prior to participating he or she should inspect the facilities and equipment to be used, and if he or she believes anything is unsafe the participant should immediately advise the staff of such condition and refuse to participate.

2. All protective gear and equipment must be designed for approved activities and be in good condition. Rocky's Skate & Sales LLC, DBA Black Mamba Skate Park staff reserves the right to refuse to allow anyone to skate if their protective gear and equipment is not adequate.

3. All users must get a WRIST BAND at the sign in desk prior to entering the ramp area.

4. Only approved activities are allowed in the ramp area.

5. Participants must wear clothing appropriate to activity including shirts and closed toe shoes.

6. No obstacles are to be brought in to the ramp area/roller rink.

7. All users agree to act in an orderly, safe and considerate manner while in the ramp area. No
"snaking." Everyone is to wait their turn.

8. Users are to immediately report any safety concerns or injuries, whether major or minor in nature to park staff. In the case of a serious injury, do not move the injured person and all activity shall cease immediately.
