Monthly Service Report - 2025
Welcome to the Northern Illinois Food Bank monthly service report submission form for 2025!
Member Agencies of the Food Bank are required to maintain and report records that accurately reflect the total amount of product received and households/persons served monthly. Monthly reports include the following information:
• Year of the reporting period
• Month of the reporting period
• Agency name and number (ie. Feeding Food Panty, 1001A). If your Agency name and number are not listed, please contact your Area Leader to be added to the drop down.
• County where your agency resides
• Whether or not your agency participates in the Emergency Food Program (EFP). If yes, then the form will be redirected to EFP form.
• Whether or not your agency is a School Based Food Pantry (SBFP). If yes, then the form will be
redirected to the SBFP form.
• Whether your agency is a meal (ie. soup kitchen) or grocery (ie. pantry) agency type
• Service number information (defined below)
• Contact information
