Locker Request Form (2025/2026)
Please use the form below to request a locker. Be sure to use your HCPSS Student Email Address.  

Requests will be processed in the order in which they are received. Locker information will be delivered to your 3rd period class starting Wednesday, September 3.

Please consider where you would like your locker to be located. Select your first and second choice location when prompted. If you're unfamiliar with the hallway locations, please use the school map to get an idea of where your locker could be located. We will do our best to accommodate your choices.
