Stakeholder CRDP Survey Questions

Each fiscal year ACRC submits its “Community Resource Development Plan (CRDP)” to DDS requesting special funding for the development or expansion of community-based services/programs (a.k.a. safety net, crisis, and support services) in order to address the unmet or under-met needs of regional center clients with challenging service needs, as well as the needs of our aging population. These services/programs are meant to reduce reliance on the placement to Porterville Developmental Center (PDC), Institutes of Mental Disease (IMD), other restrictive settings (Skill Nursing Facilities (SNF), hospitals, etc.), and out-of-state placement.

In order to assist ACRC in determining which safety net, crisis, and support services are necessary, please complete the following survey.
