W celu zapewnienia jak największego bezpieczeństwa niezbędne jest bezwzględne przestrzeganie Regulaminu, jednakże nawet przy zachowaniu wszelkich środków ostrożności bouldering może wiązać się z ryzykiem uszkodzenia ciała lub doznania rozstroju zdrowia. Korzystanie z Boulderowni odbywa się na własną odpowiedzialność wspinających lub ich opiekunów.
Akceptując warunki tego formularza oświadczam, że:

- zgadzam się, aby osoba niepełnoletnia korzystała z Boulderowni zgodnie z warunkami Regulaminu, zgodnie z którym:
- osoba która ukończyłą 14 lat może korzystać z Boulderowni samodzielnie- osoba która nie ukończyła 14 lat może korzystać z Boulderowni wyłącznie pod nadzorem osoby dorosłej- Zapoznałem się z klauzulą informacyjną dotyczącą przetwarzania danych osobowych

Statement of a Parent or Legal Guardian of a Minor
Sport climbing and bouldering are high-risk sports. To ensure the highest level of safety, strict compliance with the Rules and Regulations is essential. However, even when all precautions are taken, bouldering may still involve the risk of bodily injury or health impairment. The use of the Boulder Gym is at the sole risk of the climbers or their guardians.
By accepting the terms of this form, I declare that: - I am the parent/legal guardian of the listed minor.
- The minor has no medical contraindications to practicing climbing.
- I am familiar with and accept the Rules and Regulations of Boulder Gym CUBE and MOOD. I consent to the minor using the Boulder Gym in accordance with the terms of the Rules and Regulations, under which:
- A person who has reached the age of 14 may use the Boulder Gym independently. - A person under the age of 14 may use the Boulder Gym under the supervision of an adult. - I have read the information clause regarding the processing of personal data.
