Birthday Party Request Form

Thank you for your interest in hosting a birthday party at the Science Center of Iowa!

A 50% non-refundable deposit is required to complete your reservation.  If you need to cancel the party for any reason, we will reschedule your party for a mutually agreed upon date and time. We accept credit cards, checks or cash.  Payment in full is required prior to the start of the party.  After we receive your request form, a member of our staff will contact regarding the reservation and payment.
