ACHIEVE (Action Communities for Health, Innovation and Environmental Change) Coalition is a group of multi-sectoral partners who have a collective vision of ending health inequities in chronic diseases for African-Americans and African immigrants/refugees in Multnomah County. Since its establishment in 2009, the coalition has advocated for the use of communication campaign, policy, system and environmental (PSE) change strategies to improve health outcomes for the Black population of Multnomah County. We work collectively, to increase access to healthy foods, improve community design that connects the built environment as safe and accessible places for physical activity, offering tobacco cessation programming and breastfeeding support in the places people live, work, worship and study. The group acts as the steering and advisory committee for the Multnomah County-Racial and Ethnic Approaches to Community Health (REACH) program.
The ACHIEVE Coalition meets monthly, on the first Wednesday from 9:00 A.M. - 11:00 A.M. Along with our monthly meetings, we also share information and resources through weekly announcements .
