Collection of equality information is solely for monitoring purposes to ensure that our policies and procedures are effective.  We also collect this data in accordance with the general and specific public sector equality duties under the Equality Act 2010.  Any data you enter onto this monitoring form will only be used for monitoring purposes.
As a club participating in the EFL Equality Standard and the FA Diversity Code, we may share statistical results from our equality and diversity monitoring with the EFL and the FA.  This will always be in collated form.
The information you provide on this questionnaire is anonymous and is kept fully confidential, with access strictly limited in accordance with the Data Protection Act.  You are not required to provide your name and completing it is entirely voluntary.
