CHCPS Information Form
Capitol Hill Cooperative Play School is able to host three playgroups in the nursery of The Lutheran Church of the Reformation.  Each group is insured to host 12 children.  To participate, a child must be age 2 by December 31 of the year in which they are applying.  

Constraints on space require CHCPS to hold a  lottery in the spring to fill each group for the fall.  To attend the lottery, please complete this information form and submit a non-refundable administration fee of $25 by April 30th.  An email will be provided with the date of the spring lottery.

Please make checks payable to CHCPS and write your child's name on the check.  They can be dropped off during playgroup hours (M-F 9am to 12pm), or mailed to the church.  212 East Capitol St NE

If all play group spots are currently filled, your family will be added to a waiting list.  If families leave, invitations to new families will be extended from this list.
