Sub Hold Requests


DO NOT ENTER A REQUEST ON A CLOSED DATE.  IT WILL BE DECLINED.

PLEASE READ ALL INSTRUCTIONS -

***If you are using the MEETING/PROFESSIONAL DEVELOPMENT reason for your absence in Frontline you need to fill out this form.***

Hold Limits: 30 per group / 40 total holds per day

Dates will be released 1 quarter at a time, 1 month prior to the start of the quarter.


LARGE GROUP TRAINING/MULTIPLE SUB REQUESTS (5 or more):

1. Training groups/requests of 5 or more will receive priority approval.
2. All requests will be reviewed by the sub desk.
3. If approved, the requester will be sent an approval email and it will be added to the sub holds calendar.  If the request is denied, the sub desk will send an email.
4. The requester will be responsible for communicating to the staff that they need to enter their absence in Frontline and that all the pertinent information needs to be included in the Notes to admin.  

In the Frontline absence the Notes to Admin must include:
-Name of the event as it appears on the calendar
-HRTF
-Name of the prearranged sub (if applicable).



INDIVIDUAL REQUESTS:

1. Individual requests will be reviewed and considered after large group trainings.
2. All requests will be reviewed by the sub desk.
3. If approved, the requester will be sent an approval and it will be added to the sub holds calendar.  If the request is denied, the sub desk will send an email.
4. The requester will be responsible for communicating to the staff that they need to enter their absence in Frontline and that all the pertinent information needs to be included in the Notes to admin.  

In the Frontline absence the Notes to Admin must include:
-Name of the event as it appears on the calendar
-HRTF
-Name of the prearranged sub (if applicable).

STEP 2:  Fill out all of the questions below to submit your request. Your request will go into the Sub Coordinators queue.  

STEP 3:  Requests will be reviewed within 5 business days of submission.  If Approved, the requester will receive an approval email. If Denied, the requester will receive an email from the sub desk.

STEP 4:  Enter absence in Frontline -  

You MUST Include the following in the Notes to Admin:

-Name of the event as it appears on the calendar
-HRTF
-Name of the prearranged sub (if applicable).
