Instructions: If you would like me to write a letter of recommendation, please submit this form at least 2 weeks in advance of the deadline, but preferably earlier.

Some of these questions involve logistics, while others ask for more personal details about yourself. Include things even if you think I already know/remember the answers. I have many students and I don't want to leave anything out. The answers to these questions will help me write the best letter possible - I want you to get the job/internship/REU!
