1) You have provided technical assistance or consultation regarding a grant proposal.
2) You have provided financial or in-kind support regarding a grant proposal.
3) You or a family member have been an employee of the applicant.
4) You or a family member have gained or will gain financially if the proposal is funded.
5) You may be considered to have a conflict for another reason.
Please list any schools, districts, education service districts, community colleges, universities, or non-profits where a conflict of interest may exist. Please describe your involvement with this organization which makes you think there may be a conflict of interest. Once reviewers are selected, an official statement of conflict form will be mailed to you for your signature. If there are no conflicts of interest, please enter "n/a."
