OF102-KHSAA Official Incident Report
Use this form to report an incident that occurs at a contest unrelated to the ejection of a player or coach WHO WAS REPRESENTING A KHSAA MEMBER SCHOOL. This form will be processed and any necessary follow-up will be done by the sport administrator for that specific sport or sport-activity.  You also use this report for any out-of-state ejection (Non Kentucky Team).

When completing this report give full details such as game date, site, opponents, etc. so that we may follow-up with the school.

DO NOT USE THIS INCIDENT REPORT FOR A PLAYER/COACH DISQUALIFICATION.  USE OF105-DQ REPORT.
