While performing my volunteer service with Misfit Dog Project, I agree to adhere to the following conditions: If you are injured in any way while interacting with the animals, report it to a member of the board of directors immediately. I understand that if I violate any of the above conditions that I may be asked to terminate my service and could be asked to leave the property. *

I recognize that while performing my services in a voluntary capacity, with Misfit Dog Project, there are risks of injury, including physical harm to me. On behalf of myself, my heirs and personal representatives, I hereby release, discharge, indemnify and hold harmless Misfit Dog Project, its Board of Directors, and agents from any and all claims, causes of action or demands of any nature or cause connected with my volunteer service. Such damages or injuries might include but are not limited to animal bites, scratches, any other accidents or injuries requiring medical attention or personal property damage resulting from my volunteer service. *

I also agree to release, discharge, indemnify and hold Misfit Dog Project harmless from any and all damages to my personal property while performing my services for Misfit Dog Project in a voluntary capacity. *

By typing my name and date, I acknowledge that I have read, fully understand, and agree the terms and conditions of the foregoing liability release waiver. *
