YOU CAN FILL OUT ONE FORM FOR EACH STUDENT/STAFF FOR THE SCHOOL YEAR.

STAY AT HOME- SICK POLICY
2021-2022:

Colds, flu, and other contagious illnesses are a serious concern in a public school environment due to their ability to spread so rapidly. As we continue to navigate the COVID-19 pandemic, Middletown Public Schools strives to stay diligent in our efforts to prevent the spread of all illnesses. Parents/guardians are asked to keep their child home if they are showing signs or symptoms of illness. If your child is ill, please call or email your child’s school nurse to report the absence and to discuss return to school protocol. Clearance to return to school will be dependent on your child’s presenting symptoms and will be handled on a case by case basis according to district and Rhode Island Department of Health protocols. If your child arrives to school exhibiting any active symptoms of illness, he/she will be sent home both for your child’s own safety, but also for the safety of other students and staff within the school.

If your child is suddenly exhibiting any of the following symptoms, they should stay home and not be sent into school:
fever (temperature of 100 F or above), feels feverish/warm or has chills
cough
shortness of breath or difficulty breathing (seek medical attention immediately)
fatigue
muscle or body aches
headache
new loss of taste or smell
sore throat
congestion or runny nose
nausea or vomiting
diarrhea

Middletown Public Schools: Covid-19 Testing CONSENT:

The Rhode Island Department of Education (RIDE), in collaboration with the Rhode Island Department of Health (RIDOH), is pleased to partner with Middletown and offer participation in a COVID-19 testing program in our District. Rhode Island continues to pursue resourceful testing for COVID-19 as a foundation for addressing the pandemic in the state. As part of the state testing approach, we are continuing to offer this option for the 2021-2022 school year.

Our goal is to have everyone in our learning community BE SAFE and FEEL SAFE. Access to testing will allow us to stay ahead of any type of spread while maintaining the confidence of staff, students, and families.

The Middletown Regional School District are being provided a supply of COVID-19 Antigen/Rapid Test Cards and PCR tests. These tests have been made available to RIDOH from the federal government.  This is being offered this school year free of charge. The district aims to scale up its testing program, although future testing may be dependent on supplies. This consent will apply for all testing sessions going forward through the end of the 2021/2022 school year.

BY PROVIDING CONSENT below you are giving permission FOR YOUR CHILD TO PARTICIPATE IN ROUTINE COVID TESTING IN SCHOOL.  This form must be completed in its entirety by a parent/guardian and submitted prior to in-school testing. Once submitted, your child's name will be on the list of individuals who have signed consent to participate in any Covid-19 testing events for the 2021-2022 school year.

If you are a student over the age of 18 years old or staff member of Middletown Public Schools, you may fill out this form yourself. By doing so, you will be providing consent for yourself to participate in Covid-19 testing for the 2021-2022 school year.

Testing Procedure:
Students or staff will receive a cotton nasal swab that will be manually inserted into each nostril. Individuals may self-swab over the age of 12. This sample will then be tested according to RI Department of Health and manufacturer's instructions.

If you have any questions about the program, the testing process, or retrieving test results, please reach out to your perspective school administration and nursing staff.

REMINDER: In order for students under 18 years old to participate in Covid-19 testing in Middletown Public Schools, parent/guardian permission is needed. THIS FORM MUST BE COMPLETED IN FULL for your child to participate in COVID testing within the district.


IMPORTANT: You only need to complete this online form ONE TIME. You DO NOT NEED TO send in a copy of this form or any paperwork to the school. Anyone that has tested positive for COVID-19 in the last 90 days should not be tested for COVID during that window.
