This survey is for Collaborative
Professionals in Florida to fill out upon completion or termination of a Collaborative
Family Law matter. The purpose of this survey
is to gather and analyze data about how the Collaborative
Process is being practiced in the state and add a valuable debriefing tool to
professionals' toolboxes.
For purposes of this survey,
a Collaborative matter is a matter where, at a minimum,
(i) each client had an
attorney, and (ii) the clients signed a Participation Agreement
that (iii) contained a disqualification clause prohibiting the attorneys from
engaging in litigation on behalf of the clients. A matter does not become "Collaborative" until all clients have
signed a collaborative participation agreement.
THE TEAM SHOULD COMPLETE ONE SURVEY AT THE CONCLUSION OF EACH MATTER,
REGARDLESS OF RESULT.
ONLY ONE (1) SURVEY FORM SHOULD BE SUBMITTED FOR EACH MATTER.

1.) The total fees charged by all professionals
2.) The total hours incurred by all professionals
3.) Value of marital estate.
4.) Total net income of the parties.
This survey should

matter.
