Anonymous Reporting
This form is to be used to anonymously submit reports of school violence, suspicious activity, bullying, or other student-related issues to the school administration. 

False reports will be taken seriously to the full extent of the law. 

The entries in this form will be monitored between the hours of 8 a.m. to 4 p.m. during the school year on days when the schools are in session. All reports submitted outside of these hours or on days when school is not in session will be investigated on the following normally scheduled school day. 

Do not use this form to report issues requiring immediate assistance. Instead, call 911 or the National Suicide Prevention Lifeline at 1-800-273-8255.
