SPRING 2026 ASAP Registration
If you would like your child(ren) to participate in the Ainsworth Schools After-school Program please fill out the following form once for EACH of your children. Registration is DUE on December 19, 2025.  Registrations will close on that day and they must be registered to attend any time throughout the semester. Clubs and regular sessions will start on January 19, 2026.  We will be choosing Clubs at a later date, after we have our number of students.

6. Homework Help Club will be offered to 1st-6th on Thursdays opposite Drumming. It will run for about 40 minutes. If they have all their homework complete they will work on reinforcing math and reading skills.  *

13. The Ainsworth Schools After-School Program does NOT have a nurse on staff. The ASAP staff is NOT authorized to disperse medication. If a child requires medication during ASAP hours, it will be the responsibility of the parent/guardian. The ASAP staff will only provide medical attention in the form of soap, water, ice, and bandages.

14. An after school snack will be provided every day. Please list all food allergies your child has. If they are not allergic please state "none".

16. My Child has my permission to walk home from the After School Program. I understand that ASAP is no longer responsible for the care of my child after 5:30pm.

17. As the parent or guardian of this child I guarantee that myself or someone listed above will promptly pick up my child by 5:30pm every day they are present at ASAP. If this cannot happen I agree to withdraw my child from the program for the duration decided upon by ACS administrators.

18. If there is an emergency that prevents someone from being at the school by 5:30pm I will contact the ASAP staff as to who will be picking up my child at or before 5:30pm.

19. I agree to allow photographs and digital images of my child in ASAP publications, social media, school website etc. I give permission for my child to be photographed and/or videoed.

20. If my child should choose not to meet the expectations of conduct set by ASAP, I agree he/she will not be allowed to attend for a set amount of time decided upon by the ASAP staff and ACS administrators.

21. I will communicate with the ASAP staff of any changes in the pick up arrangements for my child.
