VOLUNTARY BLOOD DONATION FORM

Pledge :
I, hereby pledge to donate my blood at least once a year, on my birthday and I will motivate & make at least one more person take this pledge. I take the decision to be true to the cause of blood donation and donate blood till the age of 65.




प्रतिज्ञा:
मैं,अपने जन्मदिन पर, वर्ष में कम से कम एक बार अपना रक्त दान करने की प्रतिज्ञा करता हूं और मैं कम से कम एक और व्यक्ति को यह प्रतिज्ञा करवाउंगा। मैं रक्तदान के उद्देश्य के प्रति ईमानदार रहने का निर्णय लेता हूं और 65 वर्ष की आयु तक रक्तदान करता रहूंगा।
