I acknowledge I am making a commitment to a team and I will be available to attend all games as scheduled. My parent(s) is / are aware I have registered to play summer hockey and all costs associated with participating in this sport will be met by my family and we understand this is a non-refundable charge, should I decide not to play after the team is registered in the competition.
