The answers to this form will be used to inform a new style of treatment for mental health. All answers will be kept confidential between the business partners and their confederates/mentors throughout the treatment design process. After the design process, the answers will be discarded.

You can quit the survey at anytime. The only "required" questions appear at the beginning of the survey (Part 1) to make the answers easier to sort. Please be careful to protect your identity and the identity of others.

There are about 20 written-answer questions, for which you may write as little or as much as you choose. Please be mindful of your time--you may use point form. This survey is expected to take a minimum of 30 minutes.
