*Mens 2x2, Womens 2x2, Mix 2x2, Quads 4x4 (max 6 players per team with at least 1 female)**

**Please note for Quads 4x4 you will need to fill out a separate form.

FORMAT
*Double-elimination with '3 game guarantee' for all divisions with Mens 2x2, Mix 2x2 & Womens 2x2 go to cross-over match play for semi-finals & finals.
