2022/2023 Adult Coaching Form
This form is for adults who would like to coach a Community Ed Basketball team. You do not need to have a child in the league to coach. All coaches must have a current CORI form on file (new as of 1/1/21). The CORI form must be submitted with a photocopy of your drivers license. These can be submitted at any school office in the district or at the Community Ed office.
