School Sponsored Activity Parental Authorization
Pursuant to Rule 6A-10.089 (5), F.A.C., Seminole County Public Schools requires parental authorization prior to students participating in school sponsored events and activities. Each student participating in the event or activity listed below must have this form completed and submitted prior to participating.  

Hello Eagles, 

Are you in need of extra help with your academics? Are you in need of homework help? Are you in need of extra computer time in iReady, Khan Academy, or other academic websites? Join the Eagles After-School Tutoring program on Tuesday and Thursday from 4:10 to 5:30 PM. Please have your parent or guardian fill out this form in its entirety and bring ALL classroom assignments and homework to Lab E in Building 3 (on the bus loop). If you have any questions regarding the program, please contact Mr. Menos (menospz1@scps.k12.fl.us / 407-900-7875) or Ms. Sousa (sousaaz@scps.k12.fl.us / 321-578-9186). 

Students must be picked up at the front of the school at 5:30 pm. Failure to oblige may result in your child being removed from tutoring. Snacks will be provided.
