Use this form to apply for a scholarship to attend Jazz Forum Arts' early childhood educational program Jitterbugs: Jazz for Kids.
  • If the scholarship is granted, the child will be expected to attend ALL classes.
  • Financial information supplied here will be held in the strictest confidence and will not be shared with any person or organization outside of Jazz Forum Arts.
  • Financial need is the primary factor in the decision making process.
  • Scholarships will be granted on a first-come, first-served basis due to limited availability in each program.
Each scholarship awarded will be equal to the amount of 50% of one child's full program fee. If your scholarship application is granted we will reach out to you via email.
