If you are requesting open enrollment for the first time to the receiving school, please choose OPEN ENROLLMENT APPLICATION AND CONTRACT. 

If the applicant student is currently attending the receiving school ON AN OPEN ENROLLMENT STATUS,  please choose OPEN ENROLLMENT CONTINUATION APPLICATION AND CONTRACT.  Students on open enrollment status must reapply each school year to the receiving school.  Continuation Applications are due by February 1 of the prior school year.
