● Registration Forms should be completed for each independent act and for each day. 

● Your submission of this application form doesn't guarantee your participation unless approved by the organizer in the form signed confirmed agreement.

● A list of performers and entertainers should be signed individually by quoting their group name and specifying the name of  the group/individual should be received not later than July 31, 2025. 

● Recorded music on prescribed form and format shall be accepted if provided not later than September 25, 2025. 

● The Canada  Diwali and  Diwali  RazzMatazz  Festival  2025  Committee reserves the right to review the eligibility of the individuals and organizations that do not abide by established festival policies and procedures and reserves the right to consider participation at future Indo-Canada Arts Council and Canada Diwali Festivals.  

DISCLAIMER: By remaining in the artist's area and performing on stage or by entering and attending the events  &  festivals of  Canada  Diwali & Diwali RazzMatazz 2025 on either or all the dates, any person attending may be filmed, photographed, and/or recorded. 

1)  You acknowledge that you have been informed that you may be photographed and/or filmed and/or recorded and you give your irrevocable permission to be photographed and/ or filmed and/or recorded. 2) You grant your irrevocable permission for your likeness,  mannerisms and/or voice to be included in any content from such filming,  photographs, and/or recording  (and edited)  without compensation and/or credit, which content may be communicated and/or exploited in any media worldwide in all perpetuity.
