This form allows the reporting of acts of intolerance, discrimination and abuse. These actions or words can be directed at any participant: a player, a referee, a coach, or a spectator. Any comments that are overheard in a public place, or actions directed at an individual, that taunt or belittle someone else are considered abuse. This includes social media postings.
Physical confrontations between participants must also be reported. This includes unwanted touching, pushing, or striking of another individual.
