Through this form you are applying to participate in the "EDU-facilitation" Erasmus+ Training Course that will take place in Adamov, Czech Republic. Please read carefully all the questions and take time to answer them in detail. Before applying, read carefully our Info-pack. You can find more information at the project website. This application is consists of 3 parts:
1) Personal details
2) Motivation and Desired Outcomes
3) Extra questions and Declaration

Please take into consideration that selection is based on the second part of the application form: Motivation and Desired Outcomes. We do not make selection based on other details submitted in this form and based on gender, race, ethnicity, sexual orientation or religion. We treat all data confidentially and will be only used for communication with you, partner organisations and the National Agency.

By submitting this application you acknowledge the following conditions that will apply if you are selected to take part in the "EDU-facilitation" Training Course:

1. I commit myself to prepare carefully for the training course, participate in the whole training course as well as the evaluation process and follow up activities.

2. I understand that the information I provided on my special needs does not remove my own personal responsibility for ensuring my own health. Arranging a health insurance for myself for the duration of the training course is my own responsibility.

3. I acknowledge that this Training Course is a learning process and not a holiday, therefore I am committed to work on my development and manage my own learning, supported by the trainers and peers.

4. I agree with the Participant's Contribution used to support the cofounding of the training course and with the conditions of travel expenses and reimbursement, as described in the infopack.

5. I understand that my personal details will be only used for the purposes of selection, communication with the partners and reporting towards the National Agency. I understand that all data is treated confidentially.
