Please read before submitting:
1. This form must be completed PRIOR to enrolling in a non-d.tech course. By submitting this form you are saying that you have reviewed the Concurrent Enrollment Policy and want to take a course that follows all the guidelines set forth in the policy. You are agreeing to add every approved concurrent enrollment course to your high school transcript. Please complete this form for EACH class you want to take. After you complete this form, for community college classes please go ahead and also submit the Course Connection/Dual Enrollment form for the Counselor's signature.
**Note: Whether a class is placed on the d.tech transcript or not - ALL classes taken and grades are part of my permanent educational record and may be reviewed for college admissions purposes.
2. If there is a College Connection Form/Dual Enrollment Form that requires a signature from the Concurrent Enrollment Coordinator, please email it to
concurrentenrollment@dtechhs.org.
3. Student must submit proof of enrollment at the beginning of the semester.
4. After completing the class, student must request an official transcript be sent by the college directly to d.tech, please have official transcripts sent to Marci Totten:
