This form is for parental use only.  Any student who misrepresents themselves as their guardian by filling out this form are subject to suspension for forgery. Please note that attendance in school is vital for good performance.  To help your child achieve all they can, please limit tardy arrivals and early departures to only urgent medical appointments or other unavoidable appointments.
