Plant The S.E.E.D. To Health & History

In honor of Black History Month, the Delaware Breast Cancer Coalition is asking you to "Plant the S.E.E.D. to Health and History" by learning about your family history and talking to your doctor about your own personal health risks.

To aid you in learning about your family history, the Delaware Breast Cancer Coalition has designed a Family Health History Tree that you can complete and share with your doctor.

If you have complete your family tree and shared it with your medical provider, please complete this survey to enter into a chance to win a DBCC gift basket.

Please note that:
• The survey questions should take only about 5-10 minutes to complete.
• It is voluntary to participate but we hope that you will take the opportunity to contribute your opinions.
• There are no right or wrong answers. The best way you can help DBCC is to be honest in your answers.
• Question regarding personal information may be skip if you do not wish to answer. However, we hope that you answer as many questions as you can.
• Your responses are confidential and will not be shared.
