Online Learning Questionnaire and Contract
In order to participate in one of the district's online courses, it is required that students and parents review and complete this form so that we can improve the likelihood of success and to ensure we are meeting state requirements. 

A student may be prohibited from enrolling in a virtual course if the student failed a previous virtual course in the same subject during the two most recent academic years.

If taking more than two online classes, a counselor will contact the student to develop an educational development plan (EDP) to provide additional support.
